- Referral information
- Insurance information
- Medical records and operative reports, all your imaging studies (x-rays, MRI, CT and other relevant tests)
including both hard copies/CD and radiology reports from prior doctors’ visits. - List of current medications and any known allergies
- For NEW patients ONLY: fill in one the questionnaires relevant to your complaints (NECK or LOWER BACK) – see bellow
